Schedule: 08:00 am - 4:30 pm MON-FRI

Location: Fort Morgan, CO 80701.

Job Type: Contract

Contract Length: 3 months (possible extension or conversion)

Pay Range: $45- $48

*Pay offered is based on experience, expertise, credentialing, and education.

Duties:

  • Conduct patient evaluations assessing mobility, strength, balance, gait, pain, and functional limitations.
  • Assess home environment for safety risks and fall hazards.
  • Develop individualized treatment plans with measurable goals and visit frequency.
  • Complete required documentation including OASIS and physician orders.
  • Provide therapeutic exercises, gait and balance training, and functional mobility training.
  • Assist patients with transfers, stair navigation, and use of assistive devices.
  • Support daily living activities such as bed mobility and home safety tasks.
  • Adjust treatment plans based on patient progress and clinical response.
  • Educate patients and caregivers on home exercise programs, fall prevention, and safety techniques.
  • Train caregivers on proper mobility and transfer assistance.
  • Coordinate care with physicians and interdisciplinary team members.
  • Participate in discharge planning and recommend ongoing care or services.
Requirements:
  • Master’s (MPT) or Doctorate (DPT) in Physical Therapy from an accredited program.
  • Home Health experience required.
  • Strong experience with OASIS documentation.
  • Certification(s): Active Physical Therapist license (state-specific), MPT or DPT.
Additional Requirements:
  • Ability to travel daily to patient homes within assigned territory (50 miles radius).
  • Valid driver’s license and reliable transportation.
  • Comfortable working in home-based care environments.
