Patient and family education will be an important part of your work, including teaching about diagnoses, medications, follow-up care, and health management to support successful discharges and continuity of care.

You will play a key role in discharge planning and care coordination, helping to ensure that patients and caregivers understand their next steps and have appropriate resources in place. Throughout each assignment, you will uphold strict infection prevention, safety, and quality standards, following both regulatory requirements and facility policies. This opportunity allows you to broaden your clinical experience, build meaningful professional connections, and make a direct impact on patient outcomes across multiple care settings.
